Notice to Patients Regarding Privacy Incident

Great River Medical Center is committed to maintaining the privacy and security of the patient information we manage. Regrettably, this notice is regarding a privacy incident involving some of this information.

On October 13, 2016, Great River Medical Center became aware that an unauthorized person gained access to one of our computer workstations. We immediately began an investigation and engaged a leading computer security firm to assist in the investigation. Findings from the investigation show that the unknown person remotely accessed the computer workstation between October 10, 2016, and October 13, 2016. The workstation contained files that included the patient's name, address, phone number, and social security number.
This did not affect all patients, only a few patients whose information was contained on the workstation. To date, we have no knowledge that information has been used in any way.

Great River Medical Center began mailing letters to affected patients on December 9, 2016, and has established a dedicated call center to answer any questions they may have. If you believe you are affected but do not receive a letter by December 19, 2016, please call 1-855-354-4445 between 8 a.m. and 8 p.m. Central Time, Monday through Friday.

We deeply regret any inconvenience or concern this may cause you. To help prevent this from happening in the future, we are working with the computer security firm to further strengthen the security of our systems.
